His consistent domestic performances earned him a spot in the Sri Lanka Emerging Team for their 2022 tour of England. During this tour, Madushka scored 269 runs against Kent, setting a record for the highest first-class score by a Sri Lankan in England, surpassing Aravinda de Silva's 255.
Madushka made his Test debut for Sri Lanka against New Zealand on March 17, 2023. He announced his arrival on the international stage with a double century against Ireland in April 2023, scoring 205 runs and playing a pivotal role in Sri Lanka's series victory.
